[Plugin: WP Customer Reviews] Settings not saving

I entered custom fields and need to edit the text I wrote. When I make changes and click on save, my changes do not save. I am even the message that says “your changes have been saved”, but they have not been.

Also, how do I change the text on the form title, “Submit Your Review” and the text above the comment box, “review?”
