Widget Title

What happened to the ability in the widget to create a “Title” for it. When I first created the widget on the site, I had a unique widget title “Share Your Story!”, and now when I go to the widget options, the only option is to “Hide Widget Title”….Am I missing something?

What happens when you remove the widget and then add it again?
Didn’t result in any changes. It’s still without a title. I did amend the “Description” field which I can see may be where the Title is determined, but alas the widget still doesn’t have a presiding title…
I also noticed that the form that pre-exists is not translating to the widget. The form is not requiring an email address to be entered, and it’s not a visible form. However when I add the widget, it’s making the email field visible.
Are you using the latest version of WordPress? WordPress no longer uses the old style widgets but instead you add blocks to your sidebars. Maybe this is part of the problem?

Making the email field not required in the settings does not hide it in the review form. To do this, you need to use the hide option in the block settings to hide the email field.
