Filter page visitors by zip code

Hi
I have an appliance repair business, and I’m going to start offering subscriptions. Since i can only accept subscribers from the areas I service, before landing in the subscription page, I need a way for them to enter their zip code, and then be redirected to the correct page. Is there a plug-in that offers a similar feature, or can anyone give me any direction on how to implement this.
