I just learned something how this is caused and share it in this reply because I am not able to edit the original thread.
When checking the source for some reason “-100×100.png” is added behind the original icon but myimage-100×100.png does not exist causing it to display a blank container as displayed in the screenshot.
I am not using any kind of image optimizer plugin.

This happens because the achievement image has been deleted or you do not have permission in the images folder to access it.
Check that the image is loaded in the media library and also check to have read permission in the image folder of your server.
For the latter, you may have to contact your hosting provider and have them do this for you.

As for the 100×100 of the image it is due to the GamiPress settings, you can change the size of the images from GamiPress-> Settings.
In the event that you change the size you will have to use a plugin such as Regenerate Thumbnails to regenerate the images in the new size. https://es.wordpress.org/plugins/regenerate-thumbnails/

Hi @pacogon, yes the original image is available in the media and I have full server permission but the 100×100 version not available (I have not changed the image size either) and that is the issue.

It’s pretty weird, try the following:
1- Regenerate the image size with the “Regenerate Thumbsnails” plugin.
2- Win an achievement to appear in the feed.
3- Check the feed to see if the image appears.
If the achievement image appears on the achievement page (or by shortcode) it should also appear in the feed.
But if you change the image of the achievement and delete the previous one, the feed is a plain text, so you will have the URL of the image that was in that moment.
