Have you updated a plugin to the current version? If not, just do it.
My plugin is updated to latest version but still no builder.
Does it requires any plugin other than UNI CPO activated for it to show up?
Uni CPO is WooCommerce extension, so it need WC. That’s all.
Please, switch to Storefront theme an activate only WC and Uni CPO. Please, check the docs for usage tutorial.
I have activated storefront and WC, the problem still same.

Yeah because when the builder was fine, the form is also activated. Do I need to deactivate all other plugins beside WC and Storefront?
Please, check for any JS errors in browser’s console.
Also, maybe, you are using some plugin that removes WC hook needed for Uni CPO form to be displayed. So, yes, please, try deactivating all the plugins except WC
