I have the same issue – even after deactivating all plugins except DJD.
No form displays. IE, Firefox, and Chrome. All the same.

General settings are for “anyone can register” default: Contributor
User set up as Contributor
Category set up as Contributor
Page created and short code inserted [djd-site-post] (no other copy on the page)
Can you help please?
