@photocrati I haven’t because the very issue causes that settings page to not load (404s for all of the CSS and JS). Is there any way to perform the same procedure on the server side or another way?
@photocrati, no they’re not. No site URLs are defined in the wp-config.php file.

Some more information:
In WordPress’ Settings -> General section I have the following values:
WordPress Address (URL): http://www.phcfarm.com/welcome
Site Address (URL): http://www.phcfarm.com/welcome
I tried adding a trailing / but it was removed when I saved changes.
