That’s just a preview. The actual form in the live website will contain the fields you sent under the General tab.

Okay, Go to your Facebook App Page.
Click App Review tab on left side.
Check if you see “email” and “public_profile” in the Approved Items section with a Green signal icon.
If not, Click Start a Submission button under Submit Items for Approval and Select “email” and “public_profile”. Then click Add Items
On the next page click Submit for ReviewForum: Plugins

Set your fields,

Preview is just a preview of the theme, doesn’t include the form.
Under General tab, in Custom Form Section, you’ll have a table with one form, in that table under options, you’ll find the edit button.
