I think you have enabled ‘preview’ on your contact-us plugin!
Which plugin have you used for Contact Us page? Is it the default theme plugin or a contact-form plugin?

Do you have all the files inside the wp-content directory intact? Also, since you have mentioned you have backed up a complete installation of your websites. Can I ask whether you have backup (zip or compressed file) of your website?
