I was able to solve the problem with a separate plugin, Remove Dashboard Access:

You were correct that I somehow missed the setting I wanted, which is under General > Admin Bar > Apply Admin UI Customize Settings on the front end admin bar also. I just applied that setting and it looks better. However, the admin bar is still giving access to the Dashboard, which I wanted to block. I want the only item on the left side of the admin bar to be the name of the site and visit site. I do not want the user to have Dashboard access at all. Do you have any suggestions for how to make this work?
