@jayneburch first screenshot seems like some inside-wordpress issue like conflicting plugin or theme, but last looks as clearly hosting issue.
I suggest you to start from hosting issue. They should see it’s details in their logs.
Jane, also how your 404 error looks like?

Hi Jane, seems like some hosting issue.
As I remember it could be some “security” module, that treats data as a threat and gives those error.
See “Feedback” metabox and show technical details about php settings.
Pls confirm that it repeats on standard theme and other plugins disabled. If yes — then it’s clearly some hosting additional module in PHP (it’s a programmin lang for sites).

* fresh, clean standard, latest wordpress install
* wp-testing plugin
* yoast seo pluginWill it repeat then?

May be through some treaky SQL, but it’s too tech and not user-friendly.

1st of all, it’s a bit out of wordpress’s style. WordPress does not have sessions mechanism out-of-the box. So even if store results in wordpress DB, only registered respondents will be handled.
Also how do you see it? Describe all possible use cases. Do we need additional button, where and how respondent will restore it’s passing? What to do if respondent will restore passing somehow, finish it and then will try to restore once again?

I assume that this is because of other plugin, that you have enabled.
Let’s find it first.
Use “binary search” method.
1. In one tab open “Respondent’s results” page in admin area to track if issue will repeat or not.
2. In another tab open plugins page in admin
3. In 2rd tab open simple test with one question/answer
4. Then disable 50% of plugins
5. Pass test
6. Check if issue repeats at “Respondent’s results” page
7.a. If it repeats, then disable 50% of still active plugins and repeat.
7.b. If it will gone, then enable 50% of disabled plugins and repeat.When you will find those plugin — ping back here.
