You might have changed the Site URL (?)
Get access to the MySQL-database and browse table wordpr_options.
Check the ‘siteurl’ record.
Edit the path if necessary.
Customize an existing plugin or make your own.

Did you try to uncheck the option: Use CAPTCHA without PHP session ?

Check the options of the theme you’re using.
If a lightbox for media is enabled in the theme options , disable it.
