Go to Plugins —> Installed Plugins —> in your list of plugins is “WP Customer Reviews” with the gold star and the word “settings” next to it. Click on “settings” and that will take you to another screen (like what you described in your post). Now, if memory serves, there’s a place to enter your email address to get update info, etc., and right beside that are “Okay” and “No Thanks” buttons. Once you click on either of those buttons you’ll have access to the settings you saw in the screen shot.
