The same for me as well. Seems like a nice plugin but what a headache. This has to be the hardest plugin I have came across.

I have done some further debugging and this is what I have found.
If I import the database to localhost which is running mysql 5.0.27 I have no issues with the categories. When it is running on the host with 4.1.20 I do not see the categories.
I am really not sure if this is the problem since the requirements for v2.1 is MySQL version 4.0 or greater

No I checked that and replaced it. I also used the default template which already had this.
