I updated everything (all plugins and wordpress (4.6.1) to their latest versions) and tried again. Same issue was happening. Here is the debug file

More information on this: The form data is getting submitted and picked up in Pardot. So that’s good. We’re not loosing anything. It just puts it in there 6 times, which is annoying – but better than not receiving it.

It also does not affect the UX, only the form data to the database. This is happening to all of my forms on my website: suretysystems.com.

Yes I did. this is what I had to do:

1. In the 3rdParty Services Settings in WordPress, you must leave debug mode CHECKED. If unchecked it, it fails.
2. In the 3rdParty Services Mapping section, keep the field names EXACTLY as in Pardot. Examples:

• Label = Email (can be anything), Form submission Field = email, 3rdParty Field = email.
• Label = First Name(can be anything), Form submission Field = first_name, 3rdParty Field = first_name.
• Label = Last Name(can be anything), Form submission Field = last_name, 3rdParty Field = last_name.

Then save.
Then in Pardot’s Form Handlers Form Field Mapping, the names have to me exact and marked as required. Examples:

• External Field Name (for email) = email
• External Field Name (for first_name) = first_name
• External Field Name (for last_name) = last_name

Hope that is clear enough.
