I got the same issue:
– No new dynamic tags appear in Generate Tags
– If manually add to the form [dynamichidden dy-name “CF7_GET key=’name'”], the page is broken.

using: WordPress 3.8.11 and Contact Form 7 Version 3.9.1 and Dynamic Text Extension Version 2.0.1

Thanks Boban. All my posts added featured images. As I said above, the problem come from a conflict between WP e-commerce and the SilverOrchid theme. But I cannot fix this.

I found the roof: maybe a conflict occured between this theme with Wp e-commerce plugin. I deactivated wp e-commerce then the theme runs smoothly. Can anyone help me to fix this conflict? Thanks
