Nope, but I have another form with the same setup, and its working with no issues at all.
As a matter of fact, I played around with it for a bit, and it seems that form doesnt work if I have a space between tags, ie: [first-name] [last-name] wont work, but [first-name][last-name] will work.
Thanks, but tags I had there were [first-name] [last-name], I didnt put [your-email].
Interestingly enough, I have a form where I have all three tags in the Form field, and it works just fine.
Well, this is embarrassing, it turns out I had two radios with the same ID.
I had to post here first in order to figure that out 🙂
