Thank you Mikko! That code change made the results show correctly and I’ve played around with the title weight for better results.
I changed the weight of titles from 7.5 to 10 and re-indexed but it appears the results are the same.

Re: the highlighting. There is yellow highlighting, but I believe that is coming from the default theme settings, not the Relevanssi settings which are set to different colors, etc.
Re: AND vs OR: I wondered why the words “loan” or “officer” would be highlighted in the search results page if the results should only be for “loan officer” Yes, the phrase is on the same pages, but the search should just be for the phrase, no?
I will add more weight to the Titles to see how that changes things.
