Hi there, I have the same issue.
I thought it was the host company’s issue and contacted customer service, but they tried everything and no issues with their end. So they gave a conclusion that it’s the plugin issue.
Thanks for any help or explanation to resolve the issue~
I do also have this need to automatically block or add to permanent blocked IP list after they tried a certain times.
Suggestion:
At the “Dashboard – Locked IP Addresses” Tab, could you guys add a function – “Add to Permanent Block List” in the “Bulk Actions” drop list?That would be much easier and useful!

I tested it with DIVI builder activated, with “Auto” in W3TC Minify, not working, but work with “Manual”.
Currently I just keep “Manual”, will test it when I have time.

Any further thoughts to solve the issue? Should I give up W3 Total Cache?

Background, I am using Cloudflare, which I also have minify settings like JS, CSS and HTML Auto minify.
Did that setting cause the problem?
