Following your instructions of disabling all the Auto-inserts and then re-enabling them one-by-one allowed me to find the one category with an incorrect Disable setting.
Please can you clarify one aspect – am I correct in saying that all the selection criteria for Auto-Insert Disable (and Enable) are AND’ed together, not OR’s ?

Correct – I am trying to disable Auto-inserts on specific posts or pages but they still either show a Unit’s products – or worse, show “Product Not Found” for every Unit in a long list. To use one example, if you go to my News & Reviews page from my main menu, this is actually a Featured Post. I have explicitly disabled Taxonomies > Category = Featured Post on every Unit that I have – all Auto-Inserts have Featured Posts as disabled. However, if you look at my News & Reviews page, you can see that one Auto-Insert is ignoring it which I think is my Books unit.
I have screencaps that I can email to you of my Amazon Auto Links settings if that will help you ?
