Please try to deactivate all plugins except contact form 7 and redirection for contact form 7 and try again,If it works it means that there is some kind of conflict with another plugin.

1. on the “custom url” please add the complete url, you only defined “/wp-content/uploads/…”
2. you have some kind of broken ssl link on the page – make sure that all assets are imported with https – https://www.dropbox.com/s/wqw1215n32tmxmb/chrome_lKoAXUKd9G.png?dl=0
