I haven’t had these problems. Are you sure your MSSQL database tables are configured properly. Take a look at http://stackoverflow.com/questions/22899343/how-do-i-create-ms-sql-tables-for-a-fresh-wordpress-installation to see if your tables have been setup OK.
FYI The line number for WordPress 4.0 is 3220.
