To troubleshoot please test the form by submitting a mock entry and check and the form sends the request successfully (Success message is shown) in case of error please mention here, If no error is shown then go and check your landing page form submissions.

Please go to PluginOps > Settings > Enable Safe Mode > Save changes.

After this clear your browser cache and test again if the problem persists then please make sure your security plugin or firewall is not blocking the requests.

Please use PluginOps optin widget in landing page to load the popup alternatively you can also use popup placement shortcode to show the popup on a landing page.
