I want to say it is probably the theme I am using because I have tried previous versions of the Caldera Forms and all of them are having the same issue with the Caldera Widget using Conditions on a page with a Caldera Form. What do you suggest?
The older versions are fully functional I can create, clone, and import a form. Just not with version 1.2.0.

David have you had any luck with this? The site is live, but I also notice when I try to create, clone, or import a form the pop up modal is blank and the buttons are greyed out unable to click them. Here is the site.Click Me

Well I sent two emails the first one was blocked I was unaware you can not send a webpage as an attachment so I sent a second one with the code saved as text just copy the code into a IDE and save it as HTML and open it in your browser. I also sent picture.jpg files showing the error.

Let me try to at least grab the html coding for you and I’ll send it to your email. The coding will give you a good look at least.
Currently I do not have an online example you can look at in three weeks I will have though.
Okay the ReCaptcha works, but if I have a Caldera Form widget, and a Caldera Form load in the same page the conditions in the Caldera Form widget does not work.
Example: Above the header is a drop down-slider with a Quick Contact form (Caldera Form widget).

is (opt-out)The problem here is within the “Caldera Form widget.” I have a drop down slider on every page with a Caldera Form widget loaded inside. So if a page loads with a Caldera Form than I have an issue.
Maybe its my theme???
I am still testing making sure the field values are different from one another. I can do away with the condition thing bypassing it with a direct link for now. The biggest issue was the ReCaptcha and it works in the widget and with other pages containing forms.
Now if I have a page load with a Caldera Form everything works on the page. If I open the drop-down slider at the top which contains a Caldera Form widget on this same page everything works except for the conditions in the Caldera Form widget.
If I load a page without a Caldera Form just a regular page, and I open the drop-down slider which contains a Caldera Form widget everything in the widget works including the conditions.
My conclusion is an issue within the Caldera Form Widget.

BTW: Running WordPress 4.1.1 and Caldera Form v1.1.9.6

Still after the recent update two ReCaptcha will not work as well as more than one condition now. This is critical for a business contact page.

In reply to: [Caldera Forms - More Than Contact Forms] Duplicate existing forms?I exported the form and opened the .json file with a text editor such as CodeLobster (Free Editor) or Dreamweaver and changed the first line the ID”:”CF53e005fdb14ca” I changed one letter and renamed my old form then imported my new form. It Works!
Example:

Anyways I’ll continue building the forms. Thanks again for your efforts.

Version 1.1.2 is better so far it wont let me continue to the next pages without filling in the required fields first. +1 and the feedback stars are still NOT showing up. I am tempted to start looking at the code.
