- The generated ICO file is always
favicon.ico. Here the name is different. - The plugin stores all icons in in a directory named
/wp-content/uploads/fbrfg/, which is not the case here. - I can’t see RFG’s plugin markups in your code, so I suppose you disabled the plugin. This suppresses all references to any icon.
Do you have a referer in your logs? That would help you locate the broken link.

This said, the plugin works with WP 4.4. The upcoming minor version (which should be released this week or soon after) will indicate it.

When you create your icons with RFG, you have no “Do not generate the pinned tab icon” option. This icon is created no matter what.

Unfortunately, no, there is no easy workaround. Apple introduced proprietary material while defining the icon of the pinned tabs for Mac OS X Safari. So as soon as you declare this icon, the validation fails.
At least for the moment, RFG has no option to disable this icon: you have to create it.
To conclude on a more positive note: web pages are full of such validation errors. It’s always frustrating to get a validation report with red entries, but in real life, it just doesn’t matter.
