You might find some useful information by scanning your site here: http://sitecheck.sucuri.net. It’s a free malware scanner, if you’re infected it’ll show you information pertaining to the infection and the type of infection. That should help you narrow down your search.
On a side note, not upgrading because of fear that a theme will not funciton in the latest version should be cause for concern. New versions of software not only introduce new features but also provide security patches to vulnerabilities. Running out of date software because of a theme is very bad advise and should be cause for concern around the design and development of the theme.

Don’t forget to scan your local environment. Folks often forget that and its often their own environment in which they are most vulnerable.
Also see if your host will allow you to work via SFTP or SSH instead of FTP.

To be safe you could always check the status of your site here: http://sitecheck.sucuri.net. It’s a free malware scanner.
As for the file you referenced, best bet is to copy the code in the file into something like pastebin.com so that we can see the content of the file. The name of the specific file won’t tell us if it’s bad or good.

I’d recommend looking at your htaccess page again. Try scrolling all the way to the bottom and right.
I’d also recommend looking at all your htaccess files on the server, odds are they are all infected. Often case folks overlook the files in other applications and that could prove problematic.
