This will sound ridiculous but check you file permissions, I was getting a not found error (404), I set the permissions on the *.php file to 644 they were at 666, I imagine 744 would work too. fixed everything.
Quick question, I want to be able to have someone edit entries but not form templates… Is this what you are doing? I use contextual to block actual page access, but it seems gravity forms doesn’t have a moderator type access.
