There are no Privileges needed to install Wordfence.

The mirror image IP your’e seeing is a standard DNS PTR record and that’s the format that most responsible hosts use when creating those records so nothing out of the ordinary there.
The security levels will auto select options for you. If you change any of those options manually we change the security level selector to ‘custom’ to indicate that you’ve changed settings manually.

Just delete the /wp-content/plugins/wordfence directory to remove Wordfence.

Can you try to get yourself throttled via your cellphone or some IP that doesn’t matter if it gets blocked, then sign in from a different IP and check if it’s listed.

No, I dont think this is related to an infection. The error:
“DataTables warning (table id…”
is interesting. You’re using some sort of DataTables database plugin? Looks like that is what is throwing that error, it’s not WordPress or Wordfence or PHP or MySQL. But that is maybe just a symptom of table corruption.
I’d start by running a full check table on all tables in your WP database using some sort of MySQL client.

Looksl ike our host has changed their configuration. Go to your Wordfence options, then change the drop down list titled “How does Wordfence get IPs:” to “Use PHP’s built in REMOTE_ADDR”.

Hi,
That is an option listed under the Live Traffic Options and it is so that that user won’t be shown in your live traffic. Not so that their logins will be ignored.
If they’re getting locked out, please increase the number of failures and consider reducing the lockout time.

No idea, that actually looks like it might be a misconfiguration on your site. e.g. a redirect that is entering a loop.
