In WordPress wp-admin, inside Appearance -> Widgets, add a new text widget, and inside the Contents area, put this code:
[rotating_banners widget='true']
Also, make sure you configured Rotating Banners -> Group (have at least 2 Rotating Banners put at the second column: This group’s Rotating Banners.

I’m 99% sure you have PHP version 5.3 (or a version smaller/older than 5.4)
The minimum required PHP version for the plugin to work is 5.4.
PHP 5.3 is not supported anymore (the end of life was on 14 Aug 2014).
The solution is to update your php version to minimum of 5.4.
