I had the same problem. I contacted support for my hosting provider (GoDaddy) and after some investigation it turned out to be an issue with ModSecurity on the Apache web server hosting my site. I found messages in my error logs that showed ModSecurity blocking my attempts to upload images, log-in to admin, etc.
GoDaddy “whitelisted” the IP address of my computer that I use to administer the site, basically telling ModSecurity to let my specific computer have full access to administer the site. Things are now working as expected.

After several attempts with their Customer Support they eventually sent me an email saying that they had whitelisted my IP address on the server running my WordPress instance and that seems to have solved the problem.

Yes, I filed a support ticket several days ago and finally was able to convince GoDaddy that this was not a problem with my code. Standard install of 3.5.2 WordPress, and I have the same problems even with all plugins disabled.
They have handed my support case off to their Advanced Technical Support team to look at the server, etc., so I am hoping that results in the problem getting fixed.
