You are using a big image for the logo which is greater than the form’s width. Either reduce the logo width below 350px or use the below settings:
Logo Width: 310px Logo Height: 120pxThis will solve your problem.
I’m able to replicate this on my dev instance. Searching for any conflicts in functions. Didn’t find any conflicts in the first look. Will look into it deeper.

I’m not able to see a logo in chrome as well. The logo image is not found in your server (http://clinicianscorner.cmcafp.com/wp-content/uploads/2015/06/fam2.png)
Try re-uploading a new image.Let me know if you have any trouble in this.
