Go in to your dashboard and click on edit this page (at the top). Check the lower right side and see if it is selected as the featured image.
Did you include a link to your site, so that others can see the problem?
You are identifying the contact form as a paragraph. switch to html mode and remove the <p> and </p> from around the contact form link.
