I have the same problem. I deactivated the plugin and my site is now fine, but of course I need a contact form!
How are you able to downgrade to the previous version?
I am using version 2.0 which asks for Name identifier (name) – however I do not know what goes in this field. I am using Aweber, and the email address gets transmitted fine, but the name doesn’t crossover.
I actually put “{!name}” (no quotes) in the field, since that’s what I have to put in the subject of an email to get subscriber names to show up. But that didn’t work.
Any help would be greatly appreciated!
