I have tried the sizing approach and have disabled all other plugins, but I still get the same error. All posts are normal post type.

The theme is AutoFocus (http://themesupply.co/themes/autofocus/). It’s a great, but surprisingly the developers have removed its built-in image cropping and positioning feature, which is quite essential to this particular theme. They recommended your plugin but I just can’t get things the two to work together.

Thanks haochi! I appreciate the suggestion… am still wondering if there’s a more automated way.
