(You don’t have to include the entire domain, just the location from the WordPress root folder is fine.)

Lastly; I’ll experiment with the plugin as well, and see what I can determine.

So, now it begs the question… What plugin are you using to allow PHP code in posts and widgets?

Then, manually clear the browser cache.Now, go and see if your pages load properly.
If they do; then a plugin is causing some issue.
Give it a try; and let me know your results.

Try clicking “Save Buttons” on the WP Edit admin page; “Buttons” tab.
Watch for an admin notice. It should tell you if any buttons have been added or removed.
Does that remove the empty box… and still allow you to access the “advanced link” button?

Okay, I don’t think that is the advanced link button. I think that is the full screen button. I think the full screen button may have been removed in WordPress 4.3. I’m not seeing it when I load a default version of WordPress in the TinyMCE editor.
If you go to the WP Edit admin page; do you see the button for “Advanced Link” in the placeholder container? It should be there.
I’ll have to remove the “full screen” button from the code… so we don’t see that empty button in the settings page.

Well, I’m not sure what is going on there. It appears your posts/pages are not displaying on the front-end of the site.
I don’t think this has to do with the content editor; but rather something in your site configuration.
If you go to your admin panel… and click “posts” or “pages”… do you see your items listed there?

Also, manually clear and refresh your browser cache.
