That is what one might expect but there is no caching plugin! Wondering if it is something to do with caching on my own computer. Anyway, we seem to be operational now, thanks so much for helping.
I hate to risk being premature about this but just out of curiosity I tested the form again this afternoon and it seems to be working perfectly in all conditions. It is slightly alarming that I have no idea why things might have changed but hoping the status quo holds!

Thanks so much for getting back to me so quickly. On the plus side, I am very pleased not to have made an obvious mistake in the coding. Thanks to the info that it works for you, I have explored a little further and suspect that the problem may be some conflict with the membership plugin (pmpro). When logged in – as I was yesterday – the conditional fields do not work at all (all fields are displayed) but logged out (or in an incognito window) everything works perfectly. The issue for me is it needs to work properly for logged in users. Do you have any suggestions as to what I might do?
