Thank you, esmi. I had this exact problem and your solution #2 solved it for me. I found the code on the Main index template (index .php).
Even though this was my error and a wp issue and NOT a hosting error, my host solved this problem for me with in 15 minutes of submitting a support ticket on a sunday morning. I’m so impressed.
