This is the PHP code snippets plugin and the ADD NEW part. First time I am using this plugin but the site was setup by someone else. I am just looking into converting the old PHP to your new plugin and found this issue so I thought I would point it out to you.

I use Chrome Version 66.0.3359.181 (Official Build) (64-bit)
Also tried it on Firefox 60.0.2 (64-bit)
Both look the same way.
