I was getting the message “Attempt to submit corrupted post data” at the top of the form. On further investigation there had been some additional text after the form’s short code. (I have no idea how it got there) and I deleted it. It seems fine now. Thanks for your help – I was quite concerned as it asks for a lot of personal data. It may also be a firefox issue.

I did not input the data but when I look at the site on firefox another person’s data is visible (name email and healyh data). I do not know this person.

Yes 🙂 thanks – I tried all that but none of it worked – I resized it to get it to look ok but I still wasn’t happy with it so tried a few others and ended up using a different plugin that produced a pop up that I felt looked better, thanks for your help.
