What i can see at the first time is that u have additional errors on the page.

Maybe if u fix them the site ist Working? On wich page can i find the Form u are using?

Yes it is easy.

You made them to a text Field it must be a date field.
You can generate the tag in contact form 7

i installed the Twenty-Eleven default theme and added Contact Form 7 and my Plugin.

So the IE is working as aspect. The question is way your theme or WP or whatever does not show the footer.

Maybe i can have a look at the backend? I can’t see a mistake in the frontend. Maybe you have to look at the error.log from php
