Is it an issue with the plugin? Why is it referencing incorrect urls which arent in the database?
I’ve experienced this on an additional site we set live – using the Yoast Test Helper has also fixed it.
I can confirm I’ve had the same issue – running this has fixed it.
However we would have updated the URLs either by WPCLI or database replace.

It currently shows the user id, their login and their email address next to each checkbox (you could get user meta instead). You can replace everything below (but not including) <form method=”POST”> with the following and it should work.
