No I have not installed any plugin to create a stylized login page. I am using the default login page

In reply to: contact form and phpmyadmin databaseActually I tried this plugin and also I tried Formidable plugin, and data is stored in database, but the problem is that I want that data to be set clearly by order for e.g.(name, email, phone..) and than all that data to be saved in my personal email? Any Idea?
