Main Domain path – /home/user/web/mydomain.com/public_html

Are you filling up website section also while commenting?
If yes then leave a comment without your website information and are you leaving comment on the same website or you had commented on different websites and all were marked as Spam?

Try to clean your cache and load again, most web hosting company put limitation of size of images, if your image is bigger than it then it will not not, try to make image lesser than of it.
