I had the same problem, I fixed it creating by creating a parent page with the title of the category and made my pages to child pages.
eg:
country (parent page)
– USA (child page)
– Canada (child page)
the parent pages will reflect nice inside the permalinks
eg http://yoursite…/parentpage/page
