Thanks for the great question.
Looks like the ‘vacation’ word is in your Personal list. To solve the issue you have to remove this word from the list. Please follow steps,
1. Go to the Personal lists,
https://cleantalk.org/my/show_private?service_type=antispam2. Do search by the word ‘vacation’ in the Search field.
3. Remove the record for hastreiter.industries.
4. Wait 10 minutes. Submit a test request,
https://hastreiter.industries/request-time-off/Does it work?

If you have chance, please try to switch off the SpamFireWall option. The option is here,
WordPress Console -> Settings -> Anti-Spam by CleanTalk.Is the performance better while the SpamFireWall is switched off?

Here is a beta,
https://github.com/CleanTalk/security-malware-firewall/releases/download/dev-version/security-malware-firewall.zipFollow this guide to update the plugin,
1. Go to WordPress Administrator Panel —> Plugins.
2. Find the plugin “Security & Malware scan by CleanTalk” —> Deactivate.
3. After the automatic page refresh find the plugin again “Security & Malware scan by CleanTalk” —> Delete. Confirm “Yes, delete these files”.
4. Download the plugin archive from here: https://github.com/CleanTalk/security-malware-firewall/releases/download/dev-version/security-malware-firewall.zip
5. Go to Plugins —> Add New —> Upload Plugin.
6. Choose the downloaded archive and press “Install Now”.Contact us if you have any questions.
