Change folder’s permissions to allow your ftp user to delete it or contact your provider.It is still there after deactivating the plug-in because if an user wants to deactivate it and reactivate it want still to see the data.

With picture: you can preload a default image putting url in Value; ‘min,exact,max size’ are in KB; equal TO means max pixel size (width or height) for thumbnail

Thanks, finally someone told me how to reproduce it, the key was WordPress MS, since I cannot reproduce with non-MS.

Second: since v2.5.0 you can hide the username, email will be used as username.Then you can add whatever fields you want. But your question is not really clear.
