The IPN settings are correct, we had to approve the payment in paypal, this has now updated in the backend and it says paid.
Do I have to do this every time I take a payment approve in paypal? Is it not possible for this to be automatic?

Yes, great, solved – I set up two minimum spends hence the double message.
Solved for anyone else having similar issues you need to go to Catalog settings on WooCommerce change show both for Products and Subcategories.
