Hi, I’m using https://wordpress.org/plugins/permalink-manager/ in conjunction with this plugin and it works as you need it to.
I got a fatal error after updating to 1.3.8, fixed it via FTP as follows,
Download https://downloads.wordpress.org/plugin/redis-cache.1.3.6.zip and unzip to plugins on your local server.
FTP into your production server, delete the plugin files, then upload the 1.3.6 files, all working again 🙂
The form maker plugin is calling ‘form-maker/js/if_gmap_front_end.js’ – this file is probably the culprit.
