I have this issue as well.
It actually started when I updates my theme, so I contacted them, Get Bowtied, and they did inform me that it was in theme and would be fixed in their next release. I use The Retailer theme on the site, not sure you have this one or not, but thought it might help if you do!

This is what I was needing! The customer note field is there, but some of the customers where not realizing it was there, and would send a note from the contact form! Thanks for your help!
