No, technically it is from wordpress@your-website.com.
For instance my contact forms say the from address is wordpress@greenteam760.com.
That is the default.
I would try doing that. If you want to know the e-mail address of who it is from… put it in the “subject” field.
Subject: [your-name] [your-email]
Try putting a space between each. [your-name] [your-email]

Hi,
I will need you to clarify what you mean by saying they won’t show up. From what you showed me, the form shouldn’t work.
It should be as follows:
[text first-name] [text last-name] [email email] [tel telephone]
You have to define the element it will be, then you include the name. You essentially only defined the names.
